Care Plan Terms
HORMONE OPTIMISATION CARE PLAN AGREEMENT
This Agreement outlines the structure, billing terms, and scope of the Hormone Optimisation Care Plan provided by The Alpha Clinic.
By enrolling in the Care Plan, you confirm that you understand the structure of care, associated fees, and your rights regarding billing and access to services.
1. PURPOSE OF THE CARE PLAN
The Hormone Optimisation Care Plan supports structured, physician-led medical care involving assessment, diagnosis, monitoring, and treatment of hormone-related conditions.
It enables The Alpha Clinic to provide longitudinal clinical oversight, ongoing monitoring, and treatment optimisation over time, supporting symptom management, safety, and long-term health outcomes.
This Care Plan replaces individual billing for certain follow-up consultations and supports the broader clinical framework surrounding your treatment.
2. CARE PLAN FEES
The Care Plan fee is:
$119 per month, billed monthly
Billing begins following the initiation of treatment or at the transition point agreed with your physician.
The monthly fee applies regardless of the timing of individual visits, as it supports the ongoing clinical programme structure, monitoring systems, coordination, and administrative support associated with your care.
3. BILLING METHOD
Care Plan fees are billed automatically to your registered payment method.
Payments are processed securely through Stripe, our third-party payment processor.
By enrolling, you authorise The Alpha Clinic to charge the monthly Care Plan fee until you cancel in accordance with this Agreement.
You may update your payment method, view invoices, or manage your billing directly through the secure Stripe Billing Portal. Access details will be provided at the time of enrolment.
4. CANCELLATION TERMS
You may cancel your Care Plan with one month’s notice.
To avoid the next billing cycle, cancellations must be submitted at least 30 days prior to your next billing date.
Cancellation may be completed:
• Through the Stripe Billing Portal
• By notifying The Alpha Clinic in writing
Fees already billed are non-refundable, except where required by law.
Cancellation of the Care Plan does not affect your ability to access insured physician services through the provincial healthcare system.
Cancellation of the Care Plan does not guarantee continuation of prescriptions. Ongoing treatment requires appropriate clinical assessment and monitoring in accordance with medical standards.
5. WHAT THE CARE PLAN SUPPORTS
The Care Plan supports the clinical structure required for safe and effective hormone therapy and may include:
• Regular physician consultations approximately every 3–4 months (up to 4 per year)
• Ongoing biomarker monitoring and review
• Treatment optimisation and dose adjustment
• Clinical oversight between visits
• Programme coordination and clinical administration
• Support from the clinic team as appropriate
• Body composition monitoring where clinically indicated
The exact frequency and nature of services may vary based on your individual clinical needs.
The Care Plan supports access to structured follow-up care but does not guarantee unlimited visits or unscheduled urgent access outside of standard clinic policies.
PATIENT RESPONSIBILITY FOR FOLLOW-UP CARE
Patients are responsible for booking and attending recommended follow-up appointments in accordance with their physician’s clinical plan.
The Alpha Clinic may provide courtesy reminders when appointments are due; however, the responsibility to schedule and attend appointments rests with the patient.
Ongoing prescriptions require appropriate clinical assessment and monitoring. Prescriptions cannot be renewed without required follow-up.
Care Plan fees continue to apply regardless of appointment attendance. Failure to book or attend follow-up appointments does not constitute grounds for refund of Care Plan fees.
6. UNINSURED SERVICES
Care Plan fees apply exclusively to uninsured components of care, including programme structure, monitoring support, coordination, and other services not covered by provincial health insurance.
Insured physician services continue to be billed to the provincial health insurance plan in accordance with applicable regulations.
The Care Plan fee does not replace or duplicate insured physician services and is not required to access medically necessary insured care.
Enrolment in the Care Plan is not required to access insured physician services.
7. NATURE OF CARE
Hormone therapy is a medical process that requires ongoing assessment, monitoring, and adjustment over time.
Participation in the Care Plan does not guarantee specific outcomes, but it enables The Alpha Clinic to provide structured, proactive care focused on long-term health, safety, and clinical precision.
8. CLINIC RIGHTS
The Alpha Clinic reserves the right to modify the structure of the Care Plan or adjust fees with reasonable notice.
The clinic may discontinue Care Plan participation if clinically appropriate or if payment obligations are not met.
9. CLINICAL DISCRETION
All diagnostic testing, treatment decisions, and prescription management remain at the sole clinical discretion of the treating physician.
Participation in the Care Plan does not obligate a physician to prescribe specific therapies or continue treatment if not clinically appropriate.
10. PATIENT ACKNOWLEDGEMENT
When enrolling in the Care Plan, you confirm that:
• You understand the purpose and structure of the Care Plan
• You understand the monthly fee and billing process
• You understand how to manage or cancel your billing
• You understand the distinction between insured and uninsured services
• You understand your responsibility to book and attend recommended follow-up appointments
• You consent to enrolment in the Care Plan
